Sam’s Passport Inc. is a Licensed and Registered Company with the Peru Consulate. Applications can only be submitted on Mondays and Wednesdays at the. All applicants have to appear in person at the Consulate of Peru.
Sam’s passport will make sure all your documents are ready to be submitted prior to your visit to the consulate. One of our staff agents will be at the consulate to greet you and walk you through the steps of getting your visa application submitted and your biometrics completed.
We will FedEx or deliver your passport back to your address as soon as the consulate releases your passport to us.
There are different types of Peru visas to apply for and each type has its own requirements, processing time and fees. Please click on the visa type that fits your needs.
Tourist visa
Gather every required document below for this tourist visa. A missing item delays consulate filing.
Required documents
Use this list for the file. Non-U.S. citizens must add status documents with the passport.
Passport
Must be signed and valid for at least 6 months beyond the intended stay.
The applicant must submit either a
notarized Copy of Alien Registration Card (Green Card) -or- evidence of legal status in USA (valid visa plus I-94 in passport)
Visa application form
Consulate may require personal appearance in which case we will be able to walk you through that process.
NOTE: Each application must be signed in the “middle of the designated signature area.
NOTE: Right index fingerprints must be applied vertically and neat, not smeared.
NOTE: Fingerprint must be done in Black ink Only, they will not accept Blue Ink.
NOTE: Please do not leave any blanks, applications that are not fully completed will be rejected by the consulate at the applicants expense.
Photos
Provide 2 color passport-type photographs with a white background.
- Size 2 × 2 inches, full face, plain white background with no shadows.
- Eyes open and looking at the camera; neck, chin, nose, forehead, both ears, and eyes must be visible.
- Natural expression with mouth closed, no smile or frown.
- No glasses, hats, scarves, or hair clips (religious or medical exceptions need a statement).
- Use printed photos from a reputable source, home or office printer copies are not accepted.
- Invalid format photos will be rejected and delay submission.
Bank statement
A copy of the applicant’s most recent bank statement it must be three months.
Police record
A recent and valid police clearance report from the applicants local city or county.
Proof of travel & Hotel reservation
Please provide a copy of your flight itinerary and a hotel reservation.
Health record
We do NOT need a proof of a yellow fever vaccination unless you have traveled to an infected Country.

A Business Letter of Guarantee is required, to be typewritten on your company letterhead and addressed to:
“Consulate of Peru / Houston TX”.
The letter must clearly explain the purpose of travel; Guarantee financial support for the visit and return transportation; the letter should be signed by an officer or manager of the company, other than the applicant.

Please provide a copy of the letter of invitation from the inviting host or organization in Peru, stating that all expenses will be covered.
