• OR  Houston Police Report Application ( Spanish )
• Sam’s Request Form 

Fees processing time

• The processing time is a minimum of 3 – 5 working days.
• The fee for the Houston Police Report is $25.00.
• The fee for The National Police report varies from one State to the other; it ranges from $75.00 to $150.00.
