Sam’s Passport Inc. expedites second limited passport applications for frequent business travelers. If your current passport is full or your travel schedule creates conflicts, we guide you through the DS-82 process and hand-deliver your application to the U.S. Passport Agency in Houston.
A second limited passport is available to travelers who can demonstrate scheduling conflicts between their travel itinerary and visa processing. You must show that your current valid passport needs to be in two places at once.
Common situations include:
Collect every item listed in the Required Documents tab. Pay special attention to Letters A and B, which must be on company letterhead. Download and complete the DS-82 and Sam’s Request Form.
Unlike first-time applications, the DS-82 is a mail-in form. You do not need to visit an acceptance facility or have documents sealed in an envelope.
Place all documents in a FedEx Priority Overnight package to our Houston office. Include your current valid passport, the DS-82, Letters A and B, two travel itineraries, and Sam’s Request Form.

Your second limited passport ships back to you along with your original passport by the method selected on your application.

Letter A must be signed by your manager or company officer on official company letterhead. It confirms your need for a second passport due to travel schedule conflicts.
Letter B must be signed by the applicant on company letterhead. It provides your personal statement explaining the need for a second passport.

Government fees are included with your application. Since this is a DS-82 mail-in process, there is no separate execution fee.
