Visa minor authorization

• For adults traveling with minors, consent form for traveling to Brazil with a minor (both parents signatures are required)
• In addition, the application will need to be signed inside the box by both parents and notarized
• All applications for minors will require an original birth certificate and a copy of the parents marriage license.
• For parents traveling with children who are adopted, please be prepared to provide valid copies of the adoption documents.

Please provide Two (2×2) Good Quality Color Passport-Type photographs with white background.

Proof of jurisdiction

Valid Notarized Copy of one of the following: driver’s license, utility bills, lease or bank statement.

NOTE: Houston Jurisdiction – Arkansas, Colorado, Kansas, Louisiana, New Mexico, Oklahoma and Texas. – The Consulate of Brazil in Houston only accepts applications for submission on Tuesday’s and Thursday’s.

NOTE:  work visas are not jurisdictional, as long as the MRE approval has been sent to the Houston Consulate prior to submission of visa paperwork for U.S. Citizens.

For non U.S. Citizens, the applicant will need to provide a proof of U.S. status that he or she is indeed in the United States at the time of the applications submission.
